　　NOTES

　　1．本申请表应由投资者本人或投资企业的法定代表签署，或由上述两者的授权代表签署（授权代表须提交经公证的法定代表的授权书）。

　　Thepresentapplicationformshouldbesignedbytheinvestorhimself,orbythelegalrepresentativeoftheenterprise,orbyarepresentativeauthorizedbyboththeabove—mentionedtwopersons(theauthorizedrepresentativeshallsubmitanotarizedletterofauthorization.)
